Mustapha began his career in Home Health right out of school in 2016. He began as a Home Health Scheduler in a large hospital system agency and was responsible for setting up daily schedules for over 40 clinicians. He was promoted quickly to a Lead role, providing advisement, problem solving, and training new schedulers into their roles. In January 2021, Mustapha joined Eden Health as the Business Office Manager for the startup agency in Clark County, WA. In his role, he oversees HR, payroll and much of the daily operations that go into running an agency. Mustapha currently is the Business Office Manager for our Eden Health Home Health and Hospice for Oregon and Vancouver, WA. Mustapha is driven by his goal of helping his community to have access to quality care within the home setting.

Originally from Salt Lake City, UT, Chris graduated with a Bachelor’s Degree (BS) in Therapeutic Recreation from the University of Utah. Upon graduation he moved to Portland, OR, to complete his internship in Recreational Therapy at Providence Portland Medical Centers acute rehabilitation unit. Upon completing his internship, he worked at a Non-Profit committed to providing inclusive transportation options for all individuals in need. After seven years he took a position working in ADA Paratransit Operations for Medical Transportation Management Inc. In this role he quickly rose into the role as Director of ADA Paratransit Assessments and Travel Training becoming a subject matter expert. Chris has vast experience in operations management, community engagement/relations, and Paratransit. Chris has a passion for serving the underserved community and making a difference every day. As Community Liaison, Chris will promote Eden Health’s Home Health and Hospice services for Portland, Oregon. In his role he works primarily in business development by driving new business and supporting the community of Eden owners, patients and clinicians served by Eden. Chris’s focus is on creating a culture of providing first and foremost, patient-centered, quality care and building strong relationships in the community Eden serves. He lives in Lake Oswego, OR, with his wife and four daughters. In his spare time, you will find him enjoying quality time with his family, coaching his daughter’s softball team, fly fishing, golfing and traveling.

Eden Hospice understands that the experience of someone diagnosed with end-stage cardiac disease is very different than that of someone with cancer or pulmonary disease. That’s why we offer symptom management to control symptoms and promote comfort. No matter what the disease or diagnosis, we believe in improving the quality of life when quantity is limited.
Our Medical Directors focus on symptom management and will work with your primary doctors to order appropriate medications. Our philosophy embraces the idea of relieving your pain and other symptoms so that you are in control of your own comfort. Our goal is to make you or your loved one as comfortable as possible.
Eden Hospice will manage the ordering and delivery process of the necessary equipment.
MEDICAL EQUIPMENT CAN:
- Improve Mobility
- Make breathing easier
- Improve your quality of sleep and help reduce pain
EDEN HOSPICE WILL PROVIDE YOU WITH THE SUPPLIES AND MEDICAL EQUIPMENT RELATED TO THE HOSPICE DIAGNOSIS, INCLUDING:
- Respiratory equipment including oxygen and CPAP, BIPAP and nebulizers
- Walkers
- Crutches
- Wheelchairs
- Bathroom and shower aids
- Hospital beds
Respite care is provided to you when your family/caregivers need time away. Patients are placed in a contracted facility for a length of time in accordance with plan benefits (typically up to 5 days). The contracted facility will provide care with the hospice interdisciplinary members to continue making visits and maintain emergency/crisis availability.

Bereavement care is an essential component of hospice care that includes anticipating grief reactions and providing ongoing support for the bereaved for a year or more after your loved one has passed.
Grieving and mourning are normal. You may experience grief as a mental, physical, social, or emotional reaction. Mental reactions can include anger, guilt, anxiety, sadness, and despair. Physical reactions can include sleeping problems, changes in appetite, physical problems, or illness. At Eden Hospice, we are committed to providing information, counseling, and resources for any reaction you may experience.
Each of us takes their own journey through grief and healing. Allow yourself to open up to the idea that not every person experiences and deals with the loss of a loved one in the same way. As there are many cultural and or religious practices supported in communities to help those facing loss, understand that there is no “one way” or “one plan” that can work for everybody.
HOSPICE BEREAVEMENT PROGRAMS FOCUS ON:
- Helping family members understand and move forward in the grief process by enabling their expression of thoughts and feelings and helping them identify or develop healthy coping strategies.
- Helping families problem-solve around adjustment issues.
- Providing guidance about decision-making.
- Addressing social and spiritual concerns.
- Assisting survivors to adapt to an environment without the deceased.

Washington's Death with Dignity Act allows a qualified patient with a terminal illness with six months or less to live to request medication that the patient may self-administer to end their life. Eden Hospice respects patient wishes regarding end-of-life care in accordance with usual and acceptable standards of practice, ethics and applicable state and federal law, including the Washington Death with Dignity Act.
