Nicole has 15 years of healthcare experience, starting her career with Southwest Transplant Alliance where she led their tissue recovery program in Texas before moving on to Eli Lilly as a Sales Rep managing a customer base of 200 healthcare providers across NW Arkansas and Southern Missouri. After leaving Eli Lilly, she worked as Program Manager for LifeCenter Northwest where she oversaw the company’s organ and tissue recovery program for the state of Montana. As Executive Director, Nicole leads Eden Health’s Home Health and Hospice for Oregon and Vancouver, WA. In her role she works primarily in business development, operations, and supporting the overall health and wellbeing of the Eden owners and the patients served. Nicole’s focus is on creating a culture of providing first and foremost, patient-centered, quality care.

Connie Dada
Director of Patient Care Services
Connie received her RN, BSN from Wright State University, Dayton Ohio 36 years ago. Working as an RN at Riverside Methodist Hospital in Columbus, Ohio on a very busy Med-Surgical Unit, evening shift, Connie gained a vast majority of her skillset. She began pursuing growth within the field and took her first management position within Outpatient Surgery, Endoscopy and Outpatient Services at Doctors West Hospital in Columbus, Ohio. Connie was also honored to be President of the Management Association. In 1991, Connie and family moved West to California where she worked as an assistant nurse manager at Stanford University Hospital on the Orthopedic Unit. Fast forward to present, she has spent the past 18 years of her career in home health, palliative care and hospice and has enjoyed it all. It is truly her forever niche and her love for teaching, whether it be patients, families, caregivers or clinicians really shines. Connie serves as the Director of Patient Care Services for Eden Home Health and Hospice in Vancouver, WA and Oregon. Her clinical oversight leads our clinician teams, and provides quality review of all patient care. She believes highest quality of care is the priority and that is not only meant for our patients, caregivers and families, but also for our incredible team, administration and back office. Connie’s constant reminder: that if we are open to it, we can learn so much, sometimes daily from each other.

Mustapha Bah
Business Office Manager
Mustapha began his career in Home Health right out of school in 2016. He began as a Home Health Scheduler in a large hospital system agency and was responsible for setting up daily schedules for over 40 clinicians. He was promoted quickly to a Lead role, providing advisement, problem solving, and training new schedulers into their roles. In January 2021, Mustapha joined Eden Health as the Business Office Manager for the startup agency in Clark County, WA. In his role, he oversees HR, payroll and much of the daily operations that go into running an agency. Mustapha currently is the Business Office Manager for our Eden Health Home Health and Hospice for Oregon and Vancouver, WA. Mustapha is driven by his goal of helping his community to have access to quality care within the home setting.

Originally from Salt Lake City, UT, Chris graduated with a Bachelor’s Degree (BS) in Therapeutic Recreation from the University of Utah. Upon graduation he moved to Portland, OR, to complete his internship in Recreational Therapy at Providence Portland Medical Centers acute rehabilitation unit. Upon completing his internship, he worked at a Non-Profit committed to providing inclusive transportation options for all individuals in need. After seven years he took a position working in ADA Paratransit Operations for Medical Transportation Management Inc. In this role he quickly rose into the role as Director of ADA Paratransit Assessments and Travel Training becoming a subject matter expert. Chris has vast experience in operations management, community engagement/relations, and Paratransit. Chris has a passion for serving the underserved community and making a difference every day. As Community Liaison, Chris will promote Eden Health’s Home Health and Hospice services for Portland, Oregon. In his role he works primarily in business development by driving new business and supporting the community of Eden owners, patients and clinicians served by Eden. Chris’s focus is on creating a culture of providing first and foremost, patient-centered, quality care and building strong relationships in the community Eden serves. He lives in Lake Oswego, OR, with his wife and four daughters. In his spare time, you will find him enjoying quality time with his family, coaching his daughter’s softball team, fly fishing, golfing and traveling.

Skilled nursing care is provided by a Registered Nurse (RN) or a Licensed Practical/Vocational Nurse (LPN/LVN) to manage, observe, and evaluate care. Most often, skilled nursing is needed when an individual is recovering from injury or illness. Our nurses have significant training and experience in treating and educating patients within the home setting. We offer a range of skilled nursing care in treatment areas such as anti-coagulation therapy, cancer care, cardiopulmonary disease, diabetes, IV therapy, medication management, pain management, stroke, advanced wound care and more.
Physical Therapy helps you return to your prior level of function following an illness or injury, to improve your balance and strength. Your Physical Therapists will help empower you to prevent ongoing loss of independence by developing a program to keep you active and improve your physical abilities.
Occupational Therapy focuses on improving your Activities of Daily Living (ADLs), such as getting dressed, showering or preparing for a meal by helping you adapt to your environment, modify tasks, and use equipment to improve participation in every aspect of your daily life. Our Occupational Therapists will help you manage fatigue so as to conserve energy and reduce stress.
Speech-Language Pathologists (aka Speech Therapists) help you in evaluating the treatment of breathing, swallowing, speaking, cognition and thinking skills associated with communication disorders.
Medical Social Workers are available to help discover community resources and/or assistance that you may require short-term and long-term while recovering from an illness or injury.
Home Health Aides assist you with your daily activities until you are ready to be independent. Our Home Health Aides will coordinate with therapists to assist in exercises developed within your plan of care, as well as personal care, bathing/showering and grooming needs.

I would always go to Eden for Home Health. They are awesome in all fields, caring and so good at what they do! My PT was the best of the best and nursing was greater than words. Over the last 20 years of taking care of my handicapped son; by far, Eden is the absolute best! As I have aged and need help now, Eden is a dream come true.
